Bundled Payments: Adapting to Changes in the Post-Acute Sector

The combined payment pressures from at-risk hospitals, ACOs, bundlers and managed care are leading to the emergence of narrow and narrower post-acute networks. This momentum is further propelled by the CMS announcement of its goal to have 90 percent of Medicare FFS payments value-based by 2018. Right at this time, more than 6,500 physicians, hospitals and post-acute providers are making decisions about whether to become risk-bearing bundlers under a 3-year pilot. The combined market presence of bundlers and ACOs will be a tipping point for transformational change for skilled nursing facilities, home health and post-acute providers in many markets around the country. Post-acute providers must implement care redesign, reduce the total costs of care across an episode and produce optimal outcomes. There will definitely be winners and losers as post-acute providers adapt to this new managed-care landscape, and single SNFs and HHAs will be particularly challenged.
