Kenneth Boockvar, MD, MS is Professor of Geriatrics and Palliative Medicine at Icahn School of Medicine at Mount Sinai, Associate Director for Research at the James J Peters VA Geriatrics Research Education and Clinical Center, and Director of Clinical Studies at Jewish Home Lifecare, all in New York City. He studies health care delivery for older adults, focusing on patient transitions between sites of care, medication reconciliation, and changes in health in long-term care patients. Dr. Boockvar’s innovations have helped improve transitions and quality of long-term care for older adults and are widely disseminated. His nursing home ‘Early Warning Instrument,’ an assessment instrument designed to enable Certified Nursing Assistants to record non-specific changes in health in nursing home residents, is now used nationally as part of the Interventions to Reduce Acute Care Transfer (INTERACT) nursing home quality improvement initiative, and it has been adapted for commercial electronic health records. He has published pioneering studies on medication prescribing during patient handoffs, with invited presentations in national and international venues. Dr. Boockvar’s studies of electronic health information exchange (HIE) during care transitions have provided important information about HIE’s benefits and limitations. He is a primary care geriatrician and a past or present grant awardee from VA’s Health Services Research and Development service, multiple private foundations, and the National Institute on Aging.
