Active Day announced in April 2025 its acquisition of Illinois-based PremierCare Consulting, marking a strategic expansion in the intellectual and developmental disabilities (IDD) services market. Agenda Health, an Austin, Texas-based M&A advisory firm specializing in healthcare, acted as sell-side advisor to PremierCare.
Based in Naperville, PremierCare is contracted with the Illinois Division of Developmental Disabilities to provide person-centered planning and case management. Its team of Qualified Intellectual Disability Professionals and clinicians delivers in-home and community-based support, helping individuals live more independently through tailored care.
Active Day provides home and community-based services in the United States. Founded in 1988 and headquartered in Trevose, Pennsylvania, the company serves more than 7,500 members, offering healthcare and community services to people with disabilities and older adults. Active Day has pursued a dual strategy of organic growth and targeted acquisitions to expand its reach and community impact.
Active Day’s acquisition of PremierCare Consulting caps a busy April 2025, as the company also acquired South Carolina home care providers AllCaregivers, Inc. and New Generations Home Care of Florence, Inc. earlier in the month. This marks Active Day’s fourth transaction of 2025, following its earlier acquisition of Goldencare, a South Carolina-based adult day and home care provider.
The LevinPro HC team spoke with Matt Donnelly, CEO of Active Day, and Randy Decko, Senior Director at Agenda Health, about what brought these organizations together and the broader trends shaping M&A in this space.
Active Day’s 2025 acquisitions reflect a deliberate strategy to build a national platform integrating IDD and older adult services through in-home, day program and community services. This approach positions Active Day to meet rising demand for comprehensive care models across diverse markets. According to Donnelly, PremierCare stood out because of its strong reputation for person-centered planning and its alignment with Active Day’s mission of enriching community life.
“Its commitment to quality, community engagement and a highly skilled team made it clear that a partnership would enhance our shared impact,” said Matt Donnelly. “We saw in PremierCare a cultural and philosophical alignment that complements the services we provide, and an opportunity to strengthen our regional presence with a like-minded provider.”
Active Day plans to scale PremierCare’s IDD services while preserving its operational strengths.
“Our integration approach is collaborative and respectful of the legacy and strengths PremierCare brings,” said Donnelly. “We’re focused on ensuring a seamless transition for participants and staff, while also exploring ways to share resources, infrastructure and best practices across our broader network. We anticipate synergies in administrative support, training and innovation that will ultimately benefit all communities we serve.”
The acquisition gives Active Day a foothold in Illinois, where demand for IDD services is rising, driven by the state’s efforts to expand community-based care under the 2011 Ligas Consent Decree and increased funding proposed in Governor JB Pritzker’s 2026 budget for developmental disabilities services. Market growth is being fueled by expanded Medicaid waiver programs and a growing prevalence of developmental disabilities, particularly among aging populations. These trends create an attractive environment for providers like Active Day to scale community-based offerings.
The PremierCare acquisition reflects Active Day’s response to a rapidly evolving IDD sector, where providers face growing demands for personalized support, workforce stability and sustainable funding.
“By joining forces with PremierCare, we’re enhancing our ability to meet these challenges with a diversified, robust support model that reflects our commitment to inclusive, person-centered care,” said Donnelly.
Evolving regulations, including stricter compliance and reimbursement requirements, are driving consolidation in the IDD sector. Larger platforms, like Active Day, are better equipped to navigate these complexities while scaling community-based services with a person-centered approach and a coordinated service model.
Agenda Health anticipates continued momentum in IDD M&A, driven by rising demand and funding trends.
“We see continued increased velocity of IDD transactions, particularly those that deliver in-home care and other related services to its participants,” said Decko.
Surging IDD deal activity signals a market prioritizing scale and specialized care. As transactions accelerate, providers face pressure to consolidate, innovate, and adapt to meet growing demand for community-based services.
