H2 Health, a portfolio company of Grant Avenue Capital, LLC, has expanded its presence with the acquisition of Advanced Physical Therapy (APT).
APT is a physical therapy and rehabilitation practice with six locations in the Little Rock, Arkansas market.
H2 Health provides physical therapy and rehabilitation services. It operates more than 300 clinics across 13 states.
The acquisition of APT expands H2 Health into a new state and further demonstrates H2’s ability to execute growth. H2 Health is already a leading provider in states adjacent to Arkansas including more than 70 locations in Texas and more than 25 locations in Oklahoma. Terms were not disclosed.
This is H2 Health’s first acquisition of 2026, as well as its first since it purchased Greg Ott Center for Physical Therapy in 2023.
According to data in the LevinPro HC database, this deal marks the 12th Rehabilitation transaction of the year. Throughout 2025, 49 Rehabilitation acquisitions were announced.
